You will need a current referral from your general practitioner to claim the maximum medicare rebate. Please bring this with you to your appointment. Faxed referrals sent in by your GP prior to your appointment are preferable.
You will receive an SMS reminder prior to your appointment if you have provided your mobile phone number. Please reply with Y to confirm. If you need to cancel or change your appointment, please call at least 24 hours in advance. This allows us to fill the appointment time with patients on our waiting list. Fees may apply with failure to notify us due to cancellations.
Please call reception staff if you are running late, so appropriate arrangements can be made to accommodate you with minimal disruption to other patients.
All new patients must complete and sign a Patient Registration Form before the first appointment.
